Flu Vaccine - Parent/Guardian Authorization Form
Flu vaccines will be administered at school on Wednesday, October 27 at 10AM. If you would like for your child(ren) to receive a flu vaccine, please complete one form (front and back) for each child and return to the school office by Friday, October 22. Please email a copy of the FRONT and BACK of your insurance card to rkinney@sterlingpublicschools.com.
